Post-op Bariatric: IV Dextrose only — no oral glucose.
🚫 NPO / Unable to swallow — D50 50 mL IV push over 5 min (preferred) OR D10W 250 mL over 16 min. Recheck BG 15 min after dextrose. Still <70 → repeat dose → call MD for new orders.
🚨 Unresponsive — Activate RRT — Airway first. Stay. Call RRT. Notify MD. IV → D50 or D10W. No IV → Glucagon 1 mg SQ, turn on side, D5W 75 mL/hr. Recheck BG 15 min after Glucagon.
